Delta Dental is the UT provider, no forms are required for office visits. Dental insurance provides payment of 100% preventative and 80% restorative expenses in excess of $100 per family member per calendar year. There is no deductible for preventive work. Orthodontic expenses are paid at 60%, to a lifetime maximum of $1,500 per individual under 19 years of age.
