1) One original, signed and dated IRB Application and two copies including all required materials AND
2) A diskette or CD of the Application and Consent Form in MS WORD format.
EXEMPT & EXPEDITED REVIEWS
Applications accepted anytime but please allow 10-15 working days for IRB review and approval. Please submit:
1) One original, signed and dated IRB Application including all required materials AND
2) A diskette or CD of the Application documents in MS Word format.
